Waiver: It is the purpose of this agreement to exempt, waive, and relieve releases from liability for personal injury, property damage, and wrongful death, including if caused by negligence of releasees. “Releasees” include the City of Moorhead, Moorhead Police Department and any affiliates. I/we further acknowledge there may be unforeseen risks not known to us at this time. The participant and the participant’s parent(s)/guardian(s) acknowledge and understand the risks, if any, and acknowledge that included within the scope of this waiver and release is any cause of action arising.
The participant and the participant’s parent(s)/guardian(s) agree if any claim for the participant’s personal injury or wrongful death is commenced against this release, he/she shall defend, indemnify, and save harmless releases from any and all claims or causes of action by whomever or wherever made or presented for participant’s personal injuries, property damage, or wrongful death. They also acknowledge that they have been provided and have read the above paragraphs and have not relied upon any representations or releases and understand these waivers and releases are necessary to allow the participant to participate in this event.
The participant ‘s parents/guardian hereby authorize the Moorhead Police Athletics and Activities League to use photographs taken during events to be used for promotional purposes and on the website of Moorhead Police Athletics and Activities League.
